Letter of Recommendation Submission form

Use the form below to submit details that will help me write your letter of recommendation. Please include the school name, program, and date due, along with any additional information that will help me understand the best way to position and tailor the letter for your application.

In your submission, please add short sections or bullet points describing what you want highlighted, including:

• Key achievements or accomplishments you’d like emphasized

• Relevant skills/strengths (academic, professional, leadership, etc.)

• Specific experiences or projects that best represent you

• Critical points you want me to include (e.g., impact, growth, challenges overcome)

• Any context that would help me write effectively (relationships, timeline, goals)

The recommendation letter will be between 1 and 1.5 pages.

Finally, in the submission process, please clearly indicate whether you will need me to:

• Write the letter only,

• Answer specific questions, and/or

• Upload documentation (if required).

Allow 1-2 weeks to complete the letter of recommendation. Email me at pdterry@dons.usfca.edu after submitting the letter.
